GENERAL OVERVIEW:

This job is an integral part of the care delivery system working together to provide a healing environment, by placing our customers first.   The incumbent provides primary healthcare services for non-acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the Physician Assistant's scope of practice.  Work is performed in an inpatient setting dedicated to Primary Care, 24/7 coverage shifts, weekend, and on-call coverage on a rotational basis, as required.

The schedule for this position is 7 days on/7 days off


ESSENTIAL RESPONSIBILITIES

  • Provides comprehensive, evidence-based patient care for geriatric patients and those requiring inpatient primary care. This includes conducting thorough daily rounds, performing advanced medical history and physical assessments, formulating, and implementing patient-centered treatment plans, interpreting diagnostic test results, and facilitating safe and effective discharge planning. (20%)

  • Facilitates seamless communication and coordination of patient care activities with patients, families, community health services, and the entire interdisciplinary healthcare team to ensure continuity of care and optimal patient experience. Involves physician in clinical decision making of any issues that are outside the PA-C knowledge base or scope of practice. (15%)

  • Collaborates proactively with the attending physician, primary care team (if different), and specialists to develop and refine patient care plans. Seeks physician consultation and guidance for clinical decisions that extend beyond the PA-C's scope of practice or knowledge base, ensuring optimal patient safety and outcomes.(15%)

  • Prepares written prescription orders for drugs as defined in the written agreement between the PA-C and the supervising physician(s). (10%)

  • Assesses the specific learning needs of patients and their families, delivering culturally sensitive and age-appropriate education regarding their condition, treatment plan, and self-care strategies to promote understanding and active participation in their recovery. (10%)

  • Triages patient telephone calls, provides monitoring and continuity of care.  Ensures timely documentation of all pediatric patient encounters, interventions, and outcomes in the electronic health record, adhering to all regulatory and institutional standards for inpatient primary and geriatric care. (10%)

  • Participates in, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies. (10%)

  • May perform other duties such as daily rounding of patients, manage outpatient concerns and any other duty specific to specialty. (10%)

  • Other duties as assigned or requested.

QUALIFICATIONS:

Required

  • Master's degree in Physician Assistant Studies.   

  • Current PA Physician Assistant License

  • PA-C or board eligible. Ongoing successful recertification (NCCPA) as a PA-C Within 90 days of hire

  • Drug Enforcement Agency Licensure Within 60 days of hire

  • CPR - American Heart Association

  • Act 33 Child Abuse Clearance Certificate

  • Act 34 Criminal Background Clearance Certificate

  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate

Substitutions

  • Four to ten years related experience and/or training; or equivalent combination of education and experience.


Preferred

  • None

Required

  • MSN from the practicing State Board of Nursing Approved Master's Program
  • National Board Certification for Nurse Practitioners through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), National Certification Corporation (NCC), or Pediatric Nursing Certification Board (PNCB)
  • Practicing-state licensure as a Nurse Practitioner 
  • Prescriptive Authority from the practicing state Within 60 days of hire
  • Drug Enforcement Agency Licensure Within 60 days of hire
  • CPR - American Heart Association 
  • ACLS certification
  • Act 33 Child Abuse Clearance Certificate
  • Act 34 Criminal Background Clearance Certificate
  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
