Job Description

GENERAL OVERVIEW:

Coordinates and manages transplant patients' care throughout the entire transplant process.

ESSENTIAL RESPONSIBILITIES:

• Coordinates all aspects of the transplant process including, but not limited to, referral, evaluation, waitlist, perioperative care, and follow-up care.
• Performs assessment of patients' status, intervenes when appropriate and reports findings to transplant  physicians.
• Ensures that patient education is planned, developed, implemented, evaluated and documented in patient chart and transplant database.
• Coordinates and presents formal in-service education programs for professional, as well as, general audiences.
• Coordinates the donor/recipient process as a liaison with the Organ Procurement Organization (OPO), referring physicians, histocompatibility laboratory, transplant physicians, hospital staff and patients.

QUALIFICATIONS:

Minimum

• 3 years clinical experience
• Current PA licensure as an RN
• BSN; incumbents in role as of 11/1/2022 are exempt from this requirement
• CCTC required within three (3) years of start date

Preferred

• 1 year transplant experience
• American Board of Transplant Coordination Clinical Certification Examination
