GENERAL OVERVIEW:

Responsible for providing clinical and strategic oversight across the enterprise Care Connect Division for patients receiving outpatient services in all hospital systems and ambulatory clinics.  Responsible for working collaboratively with executive leadership both local and at the system level to achieve established goals.                 

ESSENTIAL RESPONSIBILITIES:

  • Collaborates with clinical leadership, physicians, and physician executives across AHN concerning the development of required capabilities. Participates in organization-wide navigational meetings, which provide oversight to the clinical navigation of patients, including strategic planning and implementation of innovation strategies. Collaborates with all disciplines to ensure patient care, access and patient experience continues to advance and integrate with system initiatives. Manages high value testing and scheduling of testing for institutes across AHN. Provides concierge services for high profile providers or as necessary for the Enterprise. (15%)
  •  Provides leadership, management and supervision to directors, managers and supervisors within scope of responsibility, ensuring hiring, development and retention of key personnel. Works closely with the VP Service Line and the Department Chiefs and Chairs in the recruitment and establishment of new physicians in accordance with strategic plans, as needed. Ensures compliance with all government and corporate policies, procedures and regulations. Ensure high quality employees are hired and retained for the team. Partner with HR to ensure processes are in place to reduce attrition and improve employee satisfaction. Ensure appropriate training to produce high quality, unit ready employees in an efficient, timely manner. Create the right cultural environment for a high performing team. Provide coaching, mentoring, discipline, and leadership to the team. (15%)
  • Both in person and virtual leadership to support improvements, concerns and opportunities that exist with clinicians involved in Care Connect.  This may require standing meetings or involvement with numerous leaders across the organization. (15%)
  • Provide clinical and practical oversight and direction for nurses navigating patients across the AHN network, including tracking metrics and patient outcomes. Collaborates and educates the navigational process across the system, extending education to patients, families, offices, and facilities. Provides clinical support and operational leadership over Care Connect Operations, both clinical and non-clinical operations to ensure best patient experience and clinical practices in scheduling. (10%)
  • Responsible for reducing barriers to care and improving coordination of services, reducing costs, and improving quality, as well as improving patient outcomes and overall patient satisfaction. Collaborate with Care Coordination Teams to assure optimization of all patients across all outpatient sites, along the continuum of care. (10%)
  • Communicate effectively through the ability to demonstrate the following: Communicate the strategy and vision of our clinical group to internal and external customers. Advise key stakeholders and build buy in regarding the effectiveness of deliverables. Utilize a balanced scorecard approach to provide regular executive performance updates. Provides consistent and thoughtful improvement initiatives to provide more efficient and cohesive operating strategies. (10%)
  • Participate in the formulation and execution of Allegheny Health Network's operations strategy and vision for the division. Work directly with senior leadership on projects to define full capabilities, project requirements and budget to ensure successful implementation. (10%)
  • Reviews and responsible for monitoring institutes yearly budget to maintain proper fiscal alignment. Perform all duties deemed necessary by leadership to ensure division success. (10%)
  • With direction from the Care Connect Vice President, will establish all elements of the AHN Care Connect patient navigational model across all AHN hospitals, facilities, and physician offices, as well as aligned and independent practitioners. (5%)
  • Other duties as assigned.

QUALIFICATIONS:

Minimum

  • Bachelor’s degree in Nursing
  • 10 years in a clinical setting
  • 3 years in a Clinical Operations leadership role
  • Valid Pennsylvania RN License

Preferred

  • Master’s degree
  • Doctorate degree
