GENERAL OVERVIEW:

This job will assist with the identification and coordination of grant and manufacturer medication assistance programs for patients of AHN Specialty Pharmacy.  The incumbent will also be responsible for processing and tracking medication prior authorization requests. The Patient Assistance Program Coordinator will be the first point of contact after referral for patients prescribed a new specialty medication by their AHN clinic.  The goal of this position is to maximize patient access to specialty medications by removing cost barriers which could slow or stop indicated therapy.  The incumbent will coordinate with prescribing office, patient and AHN Specialty pharmacy to ensure receipt of medication. The incumbent will be responsible to document all correspondence with parties involved in the patient’s EPIC chart.  Will also assist in the prescription refill process and will assist the pharmacy staff in the collection of any copay's required at the time of fill.  

ESSENTIAL RESPONSIBILITIES

  • Serve as patient and/or family advocate to access community and national resources (PACE, manufacturer free drug programs, grant applications). (30%)
  • Process/request authorizations/ re-authorizations when required. Supply appropriate documentation. (20%)
  • Assist with community outreach to establish and sustain working relationships within the network (physicians, office staff, social services, financial counselors etc). (15%)
  • On-going communication with patient to verify medication adherence and prompt identification of new barriers. (10%)
  • Ensure accreditation standards/objectives are being followed. (10%)
  • Work with health care team to reduce re-admissions and coordination of care between facilities. (10%)
  • Participation in pharmacy workflow. (5%)
  • Other duties as assigned.

QUALIFICATIONS:

Minimum

  • Master's Degree in Social Work (Employees my be grandfathered)
  • Social Work Experience in a Healthcare Related Setting

Preferred

  • 1 years of Previous Pharmacy Technician Experience
  • LSW or LCSW eligibility preferred depending upon requirements.facility

  • Excellent Communication Skills

  • Strong Computer Skills

  • Team Focused

  • Maintains Accurate Records, Data Collection Regarding Performance Measures
