Job Description : 

GENERAL OVERVIEW:

As part of the Street Medicine Urban Poverty program, this position will provide homeless services systems navigation support and case management to individuals at risk of or experiencing homelessness.

ESSENTIAL RESPONSIBILITIES

  • Collaborates closely with Allegheny Link and Dept of Human Services housing navigator unit. (20%)
  • Engages directly with emergency shelter staff to connect individuals experiencing homelessness and facilitate the referral process. (20%)
  • Serves as outreach station administrator, providing station management and oversight, including maintenance of telehealth equipment, coordination of mail. (20%)
  • Leads daily check in and shift briefing/debriefing with their team to inform QI and ensure information about participants is readily available and up to date. (10%)
  • Participates in street outreach rounds weekly for transnational benefit. (10%)
  • Provides navigation guidance, and day to day site based support to outreach and community health worker team members. (10%)
  • Serves as the main point of contact between individuals in need of resources and team members to assist them. (10%)
  • Other duties as assigned.

QUALIFICATIONS:

Minimum

  • High School/GED
  • 1 year in Public health or social services experience
  • Public health or social services experience
  • Positive individual that can maintain solid working relationships with internal and external resources
  • Patient and able to remain composed in stressful situations
  • Flexible and able to adapt to different situations
  • Represent the CIH Street Medicine team with professionalism

Preferred

  • Bachelor's Degree in Social Work, Public Health, Psychology, Counseling or Education
  • 1 year in Behavioral Health
  • 1 year with community engagement organizations
  • 1 year with Allegheny Link and housing placement
