GENERAL OVERVIEW:

This job provides direct patient and partner facing support to the clinical team. Primarily focused on pre-enrollment engagement and outreach to referred patients as well as functioning as the primary point of contact for both internal AHN and external, community based partner organizations, the incumbent must be familiar with all aspects of the program. The incumbent processes referrals from all sources, conducts preliminary background and chart reviews following approved workflows, and conducts outreach to engage the patient and responsible caregivers. Most outreach is telephonic and requires extensive computer research and documentation, however the incumbent may accompany other staff during in-patient encounters. 


ESSENTIAL RESPONSIBILITIES

  • Process patient referrals from all sources to include data migration and entry across multiple health informatics systems. (35%)
  • Conduct referred patient outreach and enrollment calls using established workflows and criteria. (25%)
  • Coordinate MIH team calendar to ensure patient and staff schedules are deconflicted and aligned with resources. (15%)
  • Conduct open source and health informatics background research to support Community Paramedic risk assessment process. (10%)
  • Assists Community Paramedics, Community Paramedic Supervisors, and other MIH clinicians with patient visits and community outreach efforts. (10%)
  • Remains current with all applicable trends and regulations within the EMS community. (5%)


QUALIFICATIONS:

Minimum

  • High School/GED
  • 1 year as a Pre-hospital care clinician (EMT, AEMT, Paramedic)
  • Emergency Medical Technician (EMT)
  • Basic Life Support (BLS)
  • Driver's License (PA)

Preferred

  • Associate's degree in Emergency Medicine, Public Health, Education, Health Science, Emergency Management, or similar
  • 1 year with a Social Services, Mental Health, or Public Health agency
  • 1 year in a Medical front office role
  • ​Basic Life Support (BLS) Instructor Certification
  • Nationally Registered Certified Coding Specialist (NRCCS)
  • Nationally Registered EMT
  • Applied Suicide Intervention Skills Training (ASIST)
